Nondiscrimination in Federally-Assisted Programs  - Complaint Instructions

Anyone who wishes to file a complaint alleging a violation of Title VI based on race, color, national origin, sex or religion, as provided by Section 504 of the Rehabilitation Act of 1973, Title VI of the Civil Rights of 1964, as amended, Americans with Disability Act of 1990; 49 CFR §27.13(b), 28 CFR 35.107(b), or other federal anti-discrimination regulations or laws has a right to file a written complaint. The Complaint Form should contain as much information as possible concerning the alleged violation, including the location, date and description of the problem. Strict confidentiality of all information provided will be maintained to the extent permitted by law. Sharing of information, including identity, will be done only as required by law or as needed to resolve the complaint.  Furnishing of the requested information is voluntary, except that the failure to provide such information may result in our being unable to process your complaint. The person filing a complaint has the choice to file an anonymous complaint; however we will not be able to follow-up with the complainant without the inclusion of a name in the complaint form and you will not have the benefit of learning the results of our investigation.

Filing:

Submit a letter or other document that describes your Title VI complaint of discrimination. Or, complete the Title VI complaint of discrimination form (English or Spanish - see below). Or, contact RNO’s Title VI/§504 Compliance Administrator to assist you in submitting a complaint. All complaints must be signed by the person making the complaint and sent to:

Complaint Information:

When filing your complaint please include the following information:

  1. Name, address, daytime phone number and email address.
  2. The basis for complaint i.e., race, color, national origin, sex, religion, creed, or disability, etc.
  3. A description of the incident that led the complaint to feel that discrimination was a factor: i.e., denial of access to services, programs, or benefits, reasonable accommodation; or language services, etc.
  4. The date(s) the action(s) in question took place.
  5. The lessees, tenants, concessionaires or contractors accused of discrimination.
  6. The names of the individuals accused of discrimination.
  7. The names and daytime phone numbers of any witnesses to the alleged discriminatory action.
  8. A detailed explanation of the sequence of events which you believe to be discriminatory.

Filing Deadline:

Time is an important factor when filing a complaint with RNO’s Title VI/§504 Administrator. Letters of complaint must be filed within one hundred eighty (180) calendar days of the date the discriminatory action took place.
